Sustainable Weight Loss After 40: What Works? with Ashley Fillmore
If weight loss feels harder than it used to—especially in your 40s, 50s, and beyond—you’re not imagining it.
Maybe there was a time when eating a little less, exercising a little more, or cutting back on indulgences was enough to see results. But add in hormonal changes, chronic stress, poorer sleep, thyroid issues, changes in muscle mass, and years of yo-yo dieting, and suddenly the old rules don’t seem to work the way they used to.
What we're left with is a lot of failed attempts, frustration, and an unanswered question: what does work?
In this episode of Thyroid-Healthy Bites, I’m joined by sustainable weight-loss expert Ashley Fillmore, founder of Metabolic Fix, to discuss a more holistic, sustainable approach to weight loss in midlife. Ashley brings nearly 20 years of experience in exercise, nutrition, coaching, and women’s metabolic health, along with a deeply personal understanding of what can happen when dieting and exercise are taken too far.
We talk about weight loss resistance, chronic dieting, metabolic adaptation, strength training, body composition, hormones, hypothyroidism, and why doing more isn’t always the answer. Most importantly, we explore how women can pursue fat loss goals without sacrificing muscle, metabolic health, or their relationship with their bodies.

What You’ll Learn in This Episode
For many women, weight loss becomes more complicated in midlife, not because they suddenly lack discipline, but because the context has changed. Hormones may be shifting. Muscle mass may be declining. Stress and sleep may look different. Thyroid dysfunction may be part of the picture. And years of restrictive dieting can influence how the body responds to yet another calorie cut.
Ashley’s approach is to step back and look at the whole person rather than immediately prescribing a more aggressive diet or exercise plan.
In this episode, Ashley and I discuss:
- Why weight loss can become more difficult in your 40s, 50s, and beyond
- How hypothyroidism and hormonal changes may complicate the picture
- Why chronic dieting and undereating can work against your goals
- What Ashley means by “repair,” “rebuild,” and “results”
- Why eating more and exercising less may sometimes be part of the solution
- Common nutrition and fitness blind spots
- Why strength training matters so much in midlife
- What “lifting heavy” actually means
- Metabolic adaptation and whether your metabolism can really be “broken”
- Why body composition matters more than scale weight alone
- Other ways to measure progress besides the scale
- Where GLP-1 medications may fit into the bigger picture
- What to look for in a coach or practitioner when you need more individualized support
Why More Isn’t Always Better
One of the recurring themes in this conversation is that many women respond to stalled weight loss by doing more: eating less, adding cardio, training harder, or tightening the rules. The standard advice to eat less and exercise more is, after all, what many of us are told by our doctors.
But Ashley often sees women who need the opposite. Her Metabolic Fix approach begins by assessing factors such as nutrition, exercise, labs, hormones, sleep, digestion, stress, and biofeedback before determining whether someone is ready for intentional fat loss. Usually, they're not.
For some women, the work begins by rebuilding a healthier foundation through adequate nutrition, strength training, improved recovery, and support for underlying health concerns. This approach is baked right into Ashley’s Repair → Rebuild → Results framework.
Metabolism, Muscle, and the Scale
We also dig into some of the confusing language surrounding “metabolic damage,” metabolic adaptation, and the idea of a “broken metabolism.” Ashley explains why long-term undereating and repeated dieting can influence metabolic function, while emphasizing that these adaptations are not necessarily permanent.
Muscle is another major piece of the conversation.
Rather than focusing solely on losing pounds, Ashley encourages women to consider what they are losing. Preserving muscle while reducing body fat is very different from simply making the scale move downward. That’s why measurements, clothing fit, strength, energy, body composition, and other forms of biofeedback may tell you much more than scale weight alone.

Final Thoughts
If there’s one thing I hope you take away from this conversation, it’s that sustainable weight loss in midlife usually requires more nuance, more wisdom—not more punishment.
There’s nothing wrong with wanting to lose unwanted body fat. But how we pursue that goal matters. For many women, especially those navigating hypothyroidism, perimenopause, menopause, or a long history of dieting, the answer may not be another restrictive plan or harder workout. Sometimes the more productive path is to rebuild the foundation, preserve muscle, support metabolic health, and find an approach that actually fits your body.
I loved this conversation with Ashley and her emphasis on sustainable, individualized care. I hope it leaves you feeling more informed, more hopeful, and better equipped to think beyond the scale.
Cheering you on,
